Anthem Provider Lookup

On the left hand side, use the “Search as a Member”, option and enter the 3 digit alpha prefix as follows:

• YTO – For Anthem off-exchange individual/family plans
• YTY – For Anthem on-exchange individual/family plans
• YTB – For Anthem Healthkeeper plans
• YTN – For grandfathered Anthem PPO plans

Search by Provider Name or Provider Type, enter your Location, and choose a Plan.

• For an HMO or POS plan, choose Blue Choice HMO
• For a PPO plan, choose BluePreferred

• ACA Plans – Not Tax Credit Eligible (OFF exchange)
• ACA Plans — Tax Credit Eligible (ON exchange)

The OFF exchange network is called United Healthcare Choice Plus

The ON exchange network is called United Healthcare Compass
