About 2018 Anthem Health Insurance
If you live in one of the 68 Virginia cities or counties shown below, you can enroll in an Anthem individual/family health insurance policy in 2018.
Anthem offers only HMO (Health Maintenance Organization) plans in 2018. You can choose between bronze, silver, and gold plans. There is also a catastrophic plan if you are age 30 or younger.
There is one HSA-compatible Anthem plan in 2018 – the Bronze 4900 for HSA. This plan is offered both on and off the exchange.
Anthem plans are sold both on and off the exchange. This means if you are subsidy eligible, you should buy your Anthem plan on the exchange to receive help with the premiums.
It is important to check if your provider is in network, because HMO plans do not cover out-of-network care (except in the case of a true emergency). Please note, providers can — and do — leave networks at their discretion.
Anthem also offers dental and vision coverage.

- To apply online for an Anthem ON EXCHANGE plan in Virginia, click here. Enter your Zip Code, Name, Email, and Phone Number, and click on “See Plans and Prices” to find a plan. Enroll and secure a subsidy.

- Visit www.anthem.com/findadoctor and search using the Pathway X Tiered Hospital Network.
